Count me in as one having the same symptoms of the OP.... only exception.. I have he official RPi5 Power Supply.
Count me in as one of these.
This is what I got:
LED Warning Flash Codes 4 long, 5 short = Fatal Firmware Error
Ran fine for an hour. shut it off orderly.. Next day.... the Boot failure.... Tried a brand new 32GB SanDisk Ultra today... Same results. Verified HDMI cables are good, keyboard/mouse is good, RPi5 powersupply is good (all of this works with the 4Bs flawlessly).
I tried reflashing the firmware using a Bootloader recovery SD Card via the Raspberry Pi Imager...
Results, Did not appear to work. Attempting the Boatloader Recovery yielded a solid Green Screen on the Monitor (which was more than I got before) and rapid beats of the green LED (apparently the 10 rapid... which means Halted)...
So,. I removed the SDCard... and for fun tried rebooting with no media (this has never worked before, but hey, maybe that recover did something after all?).... Bingo. I got the a full Boot menu on the Monitor.. I want to emphasize that booting with no media (nothing in USB, nothing) had never worked before... same ol' Fatal Error LED flashing sequence.
Feeling encouraged with the monitor, keyboard and mouse working... I instructed it to connect to the Internet via the connected Ethernet Cable and Download Pi Installer. Then I attached a USB to SDMicro Adapter w/ a new 32GB SDMicro Ultra card and downloaded Bookworm to it..
Install completed and it successfully booted to Bookworm. I then shut it down... Removed the SD Card from the USB-SDMicro adapter and placed it in the SDMicro slot directly on the RPI5... Turned it on.. All Good.
So my sense is that the Bootloader did get repaired/recovered .... Who knows? I'm happy now... I really thought this brand new RPI5 was bricked, but all appears good. Now I'll move forward with the NVMe install and begin booting off of that.
