You'd be amazed how many mains sockets have loose wires, in the UK I've never found a house with less than two sockets with loose connections, in one instance it was every socket in the house. I'm not talking about the screw needing a bit of a pinch, these are sockets where the wires could be pulled out or would shuffle back and forth with a reasonable amount of force.
How could the house or which mains socket used affect the ability of the power supply to negotiate 5A mode on the USB side with the Pi?

Noise does wonderful and unexpected things. The best one I came across was the telephone ringing when the oven was switched on, I was looking out for Candid Camera's cameras right from the time I got the phone call, especially as it was "demonstrated" to me over the phone by my electrician mate and he was insisting I came out even though I wasn't a domestic electrician.
