Use an official OS and boot your Pi. If the problem persists report back. If not yoi know it's related to your OS.

Sorry, didn't bother reading yoir entire post as there are too many references to ChatGPT 'results'.
The word "Chatgpt" is literally mentioned twice in my previous post, although I understand it was a bit long to read. In my original writeup I detail how the same issue occurred in Pi OS and LibreElec running on various different devices (USB Stick, SD Card, NVMe drive). Are Pi OS and LibreElec considered an official OS? If not, which OS should I try? The problem started out of the blue and was working fine for months using LibreElec, now the problem occurs no matter what I do.
take a fresh uSD with latest Raspberry Pi OS - https://www.raspberrypi.com/software/

Disconnect additional hardware - in particular USB drives as you may have one that back powers your Pi - and boot the Pi with only keyboard/mouse/monitor attached.
Report back on the fan behavior.
