I understand where you are going this with and, yes, it is in a case - bought from The PiHut and is one of the few (if only) production cases to support the TVHat and provide an exit point for the antenna connection.
The case itself is acrylic and holes on the side to provide access for power, HDMI & 3.5mm are quite a bit larger than the ports so there's no obstruction for the connectors as they are plugged in.

This is the v2 edition that has a different side panel to provide compatibilty for both Pi3 & Pi4 models - I think there may have been an interim version with interchangable side panels, depending on whether it was being used with Pi3 or Pi4, but the original model I have is not too dissmilar with just a slightly smaller hole for the single full-sized HDMI connector. However, like with this image the whole port and even some of the pcb is exposed preventing obstruction.
