Hi Jason, your case looks like it pretty much covers the pi, interesting that you find the bluetooth was affected by it.
I too don't want fan noise; on my pi 5 I am using a more "skimpy" metal heatsink case that has a lot more gaps around the edges. Its mounted with the top clamped to the metal plate behind the display.

After I fitted it I tried the connections speeds:
wired connection download 37Mbps, upload 9.2Mbps
wireless connection download 6.4Mbps, upload 9.3Mbps
Didn't do the same test with the naked pi5
I use a bluetooth headset with it and haven't noticed any problems.
Perhaps this design has enough air gaps to block the radio signals less?
